Why Start a 4-H Club?

ADULTS - Extension adult volunteers who plan on working with 4-H'ers age 19 and under OR adults who have severe mental physical or emotional disabilities MUST COMPLETE MSU EXTENSION VOLUNTEER APPLICATION FORMS before entering Best Friends 4-H Club Canine Classes.

As with any establishment, REGISTRATION FORMS MUST BE FILLED OUT.  And Best Friends does have its share of informational paperwork.  Each year current members and leaders must fill out six to seven (6 to 8) ENROLLMENT FORMS.  Those forms are...

  1. 4-H member & Volunteer Information Form (Good for only one year of membership)
  2. 4-H Parent Information (Leaders do not need to fill out this form)
  3. Medical Treatment/Media Release Authorization
  4. Best Friends 4-H Club Member/Leader Registration Form
  5. Best Friends 4-H Club Website Permission Form
  6. Best Friends 4-h Club Parent/Guardian - Adult Member Informed Consent and Release of Liability Agreement Form.
  7. Berrien County MSU Extension 4-H Youth Development Participant Fee Scholarship Application
  8. AND (if you have a canine joining the club) a CANINE REGISTRATION FORM.
